Join us for our Back To School Health Drive all August long for $35 checkups and school physicals for school aged children. Appointments are recommended, with limited walk-ins welcome. Don't forget to bring your insurance card, ID, Birth Certificate, and/or Shot records (if possible)!

We are a Federally Qualified Health Center (FQHC) located in Miami-Dade County. CHC’s provide high-quality care at affordable prices. We offer services based on a sliding fee scale to ensure that we serve members of the community regardless of the ability to pay. In addition to primary and specialty medical care, we offer a variety of support services.
