Empower U is pleased to offer free monthly Miami-Dade Transit passes to eligible patients. These passes are valid for the entire month you receive them and can be used on all Miami-Dade transit services, including the Metrobus and Metrorail.

How to Request Your Pass

Simply ask our Front Desk Staff to initiate your request.

Eligibility Requirements

To fulfill your request and verify your eligibility, please be prepared to provide:
 • Photo ID
 • Social Security Card
 • Proof that your household income is no more than 225% of the Federal Poverty Level

Please Note: Passes are limited and distributed on a first-come, first-served basis.

Availability:
Transportation services are currently available for Empower “U” Inc. appointments only, including:
 • Primary Care
 • Dental
 • Behavioral Health
 • Optometry

Requirements:
Please note that our clinic’s transportation service is considered a secondary option.
 • Patients should first contact their insurance provider to verify whether transportation is covered for their medical visits.
 • If transportation is not covered by insurance, Empower “U” Inc. will provide transportation as a backup service to ensure patients can attend their appointments.

Scheduling Requirements:
 • Patients must request transportation when scheduling their follow-up appointment or at least 48 hours before their visit.
 • Requests submitted after this timeframe may result in scheduling conflicts or service disruptions.

How Can I Receive Diapers For My Child?

As a service to our community, Empower U Community Health Center provides essential supplies to our patients.

• What's Available: Diapers (various sizes), baby formula, wipes, adult diapers, school supplies, clothes, food items, and car seats.
 • Who Qualifies: All Empower U patients. 
 • How to Receive: Ask a member of our Front Desk Staff for your monthly distribution.
Please Note: Supplies are limited and distributed on a first-come, first-served basis each month. We are unable to reserve diapers or any other items for you or your child.
