Primary Medical Care
ADULT MEDICAL CARE
Adult Medical Care services include physical examinations, screenings for blood pressure, cholesterol, diabetes, cancer, asthma, HIV, COPD, coronary health disease, and Hep C. Annual and routine immunizations and vaccines are provided.
WOMEN’S HEALTH CARE
Women’s Health Care services include gynecological, family planning, health education, reproductive health screenings, PAP smear, colonoscopy, breast cancer screening, HPV immunizations, and routinized HIV and STI screenings. Obstetrical services are provided though referrals to our Health Center Partners.
PEDIATRIC AND ADOLESCENT CARE
Pediatric and Adolescent Care services include hearing, vision, dental, blood lead screening, immunizations and adolescent care. For eligible patients, free vaccines are available. Licensed Clinical Social Workers provide behavioral health services to adolescent patients and psychiatric referrals as needed.
Gender affirming Care (Transgender Services)
Gender Affirming Care and Transgender Services include primary medical care, hormone replacement therapy evaluation and monitoring, pre-exposure prophylaxis (PrEP) referral, anal and cervical PAP smears, other reproductive health care, routinized STI testing, and more. Our medical professionals and case managers are trained and sensitive to the needs of transgender persons to ensure we provide quality of care to all persons on the gender continuum. Additionally, we offer social support services, on-site, and name change services referral to ensure a wrap around experience for our transgender clients.
SAME GEnder LOVING Services
Same Gender Loving services include primary medical care, pre-exposure prophylaxis (PrEP) referral and services, anal PAPs, routinized HIV and STI testing, and more. Our medical professionals and case managers are trained and sensitive to the needs of same gender loving persons to ensure we provide quality of care to all persons regardless of who they love. Our on-site social support services include community outreach, off-site social events, adult sex education, and more. We strive to provide a comfortable, educational, and supportive wrap around environment addressing the unique needs of same gender loving people.
BEHAVIORAL Health Services
On-site Behavioral Health and Substance Abuse services are provided by our Licensed Clinical Social Worker to anyone who requests counseling. Referrals are available for psychiatric treatment and residential substance abuse placement.
NUTRITIONAL COUNSELING CARE
Nutritional Counseling services are provided by our Nurse Practitioners and Dietitian who provide education to clients on how to meet their individualized nutritional needs when faced with diabetes, cholesterol, heart disease and obesity.
HIV TESTING AND COUNSELING
HIV and STI screenings are included in the patients’ visits. The patient has the right to “OPT Out”. To “OPT Out” let the Nurse know if you do not want to have any of these test. HIV Testing and Counseling Services are provided on our mobile testing van and in the office. Couples, partners, and friends may test together. Test results can be provided in 20 minutes and have 99% sensitivity to detecting HIV.

SEXUALLY TRANSMITTED INFECTIONS (STI)
STI Screenings include syphilis, chlamydia and gonorrhea. There are treatment options available. Most STIs, including HIV, are required to be reported to the Florida Department of Health.
Hepatitis C Screenings are available. Hepatitis C is curable and additional treatment options are available.
ORAL HEALTH SERVICE/
Oral Health Service/Dental Service Referrals are coordinated by our Case Managers through our agreements with other Federally Qualified Health Centers (FQHC). The referrals are made for specialty care, emergency services, dental care examinations, cleanings, x-rays, fillings, prosthetics and treatment of gum disease.
Medical Case Management Services include referrals, low income eligibility and a wide variety of support services in the community to help assist clients with meeting their needs. Medical Case Management plays an essential role in assisting clients with developing and adhering to a health care plan.
Linkage Case Management Services include assisting clients establish long term connection to medical care and additional support services. Our Linkage to Care Coordinators help clients with benefit applications such as the Affordable Care Act insurance applications, ACCESS Florida applications, linkage to HIV primary care and other benefits.
PRE-EXPOSURE PROPHYLAXIS (PREP)
Empower "U", Inc. Community Health Center provides PrEP as a prevention option to reduce the risk of sexually acquired HIV infection in HIV negative adults at high risk. The goal of PrEP is to prevent HIV infection if you are exposed to the virus. If you are at ongoing risk for HIV exposure, speak to your doctor about PrEP.
Empower "U", Inc. Community Health Center is an official condom distribution center for The Florida Department of Health. Approved organizations and individuals can pick-up free condoms at our location. Please sign up with the Miami-Dade County Health Department to become an approved condom site. It is important to increase condom availability to reduce STI transmission like chlamydia, syphilis, gonorrhea and HIV in the community.
EARLY INTERVENION SERVICES
Early Intervention Services include outpatient group and individual counseling and group interventions for young same gender loving men. Interventions focus on individuals at risk for substance use and HIV transmission.
Emergency Utility Assistance is provided by funding from Broadway Cares, Equity Fights AIDS (BCEFA) to help eligible needy patients with emergency financial assistance to pay utilities.
Daily, Weekly, and Monthly passes available for patients who meet low income eligibility requirements.
FLORIDA AIDS DRUG ASSISTANCE PROGRAM (ADAP)
ADAP helps eligible patients with HIV to get life saving medication and can help to pay health insurance premiums and insurance copayments.
HOUSING OPPROTUNITIES FOR
PERSONS WITH AIDS (HOPWA)
HOPWA is a housing program that assists eligible individuals who have HIV and an AIDS diagnosis to secure and maintain housing stability. Availability is contingent on funding and placement on the wait list. Waitlist placement is managed by the City of Miami Department of Community Development HOPWA Program.
NAVIGATORS AND CERTIFIED APPLICATION COUNSELORS
EUCHC trained Navigators and Certified Application Counselors assist patients with health care exchange insurance enrollment. We provide information about insurance plans on the government exchange, assist with enrollment in Medicaid, Access Florida food stamp program and other benefit programs for low income individuals.
